SNOO Smart Start Pilot Program
The SNOO Smart Start (SSS) Program is a turnkey solution designed for Managed Care Organizations (MCOs) and Integrated Health Networks. SNOO Smart Start integrates the tangible support of a responsive infant bassinet with a text messaging platform that connects families to the resources and services they need.
The SNOO Smart Start Program is customized to meet patient and health insurer priorities.
Value-based interventions may be used to address gaps in healthcare, improve health outcomes, lower costs, improve member engagement, enhance member satisfaction, and to increase HEIDIS Scores.
SNOO Smart Start Objectives
- Reduce infant fussing, enhance infant sleep, and ensure safe sleep practices consistent with AAP guidelines. This is intended to reduce unsafe sleep behaviors (such as prone sleeping, bed-sharing, and bulky bedding)
- Achieve high caregiver satisfaction with the products and services provided
- Foster deep, meaningful engagement with caregivers to support infant and family well-being
- Improve engagement with providers and encourage timely postpartum care, evaluation, and support of mothers and routine wellness care of infants
- Provide referrals/support for families with high health-related social needs
- Empower caregivers, increasing their self-efficacy and confidence in caring for their infants
- Gain insights into the overall health and mental well-being of families post-NICU, ensuring continued care and support
- Improve infant and parental well-being and identify concerning health trends among post-NICU infants and their families to reduce avoidable hospitalizations and healthcare costs
